Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage coordinated care for ROWI clients under the supervision of the Program Director. The role includes orientation, transportation, doctor visits, and scheduling to ensure high-quality care across programs.

The ideal candidate will have at least a high school diploma, 1 year in a therapeutic setting with youth, and driving experience. This position requires professional communication and strong organizational skills.

Qualifications

• Minimum of a high school diploma; Bachelor's degree preferred.
• Minimum of two (2) years driving experience required.
• Minimum of 1-year working with kids in a therapeutic setting.
• Experience with junior high or high school students is preferred.

Responsibilities

• Coordinate individualized service including orientation, transportation, doctor visits, and schedule.
• Ensure programs and services are implemented to high standards and clients receive quality care.
• Document shift reports and share pertinent information with the team within 24 hours.
• Administer and submit documentation in EMR for client Urine Analysis and other assessments.
• Serve as a role model and maintain confidentiality and professional boundaries.
