Responsibilities

• Coordinate individualized service, orientation, transportation, doctor visits, and schedules.
• Ensure programs are implemented to the highest standards and clients receive quality care.
• Supervise clients and ensure attendance at designated groups.
• Conduct client orientation on admission day and perform screenings as directed.
• Maintain compliance with standards and document shift reports in EMR within 24 hours.
• Coordinate transportation and document in EMR; drive clients as needed.
• Assist with facility cleanliness and coordinate client meals.
