Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage the care coordination for ROWI clients, including orientation, transportation, doctor visits, and scheduling. You will supervise clients, ensure attendance at groups, and facilitate activities under the direction of the Facility Manager.

You will conduct client orientation, perform screenings, ensure Joint Commission compliance, document in EMR, and maintain confidentiality with a professional, caring approach.

Qualifications

• Minimum of a high school diploma; Bachelor’s degree preferred.
• Minimum of two years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Ability to communicate with people from diverse educational and cultural backgrounds.
• Must cope constructively with stressful situations.
• Strong written and verbal communication skills; excellent interpersonal skills.

Responsibilities

• Coordinate care for ROWI clients including orientation, transportation, doctor visits, and scheduling.
• Ensure clients attend designated groups and supervise them throughout programming.
• Facilitate assigned groups and activities as designated by the Facility Manager.
• Conduct client orientation on admission, including screenings.
• Ensure compliance with Joint Commission Standards.
• Document shift reports and EMR entries; share pertinent information with the team.
• Coordinate transportation schedules and drive clients as needed.
• Maintain confidentiality and professional interactions; complete tasks timely.
• Attend designated staff and in-service meetings and other duties as assigned.
