Area Substance Abuse Council Inc (ASAC) is seeking a full-time Care Navigator in Linn County to provide care coordination and supportive services to clients in our community.
You will use an intensive case management model to link clients to childcare, transportation, housing, employment, and education, helping them overcome barriers to long-term recovery while documenting progress in the electronic records system.

As a full-time Care Navigator in Linn County, your role will be to provide care coordination and supportive services to patients in the area. You'll utilize an intensive case management model to link patients to services such as childcare, transportation, housing, employment, and education in order to help patients overcome barriers to their long term recovery.
Qualified candidates will have a bachelor’s degree in a human services field and one year of related work experience. This position requires a valid driver's license and the ability to pass an extensive background check.
ASAC offers a competitive compensation with a starting wage of $22.71. Additional compensation is available for candidates with extensive experience.
