Job summary

Area Substance Abuse Council Inc (ASAC) is seeking a full-time Care Navigator in Linn County to provide care coordination and supportive services to clients in our community.

You will use an intensive case management model to link clients to childcare, transportation, housing, employment, and education, helping them overcome barriers to long-term recovery while documenting progress in the electronic records system.

Qualifications

• Bachelor’s degree in a human services field and one year of related work experience.

Responsibilities

• Review medical and mental health assessments and assure necessary connections and follow up with providers.
• Meet with patients during treatment; develop work plans to remove obstacles and record progress; collaborate with clinical staff to identify needs.
• Develop and maintain relationships with community providers and resources in the region.
• Provide support for child care, housing, benefits, job search and other stabilization services; schedule appointments and referrals.
