Job summary

Area-Substance-Abuse-Council is seeking a full-time Care Navigator in Linn County to provide care coordination and supportive services for patients in the community. You will use an intensive case management model to link clients to services such as childcare, transportation, housing, employment, and education to overcome barriers to long-term recovery.

Qualified candidates will have a bachelor’s degree in a human services field and at least one year of related experience.

Qualifications

• Bachelor's degree in a human services field.
• 1 year of related work experience.
• Valid driver's license and ability to pass extensive background check.

Responsibilities

• Provide care coordination and supportive services to patients in Linn County.
• Use intensive case management to link patients to services (childcare, transportation, housing, employment, education).
• Develop work plans with patients and track progress with clinical staff.
• Build relationships with community providers and resources in the region.
• Document interactions in electronic records and report patient participation.
