Job summary

HealthRIGHT 360 in San Francisco, CA is seeking a full-time Patient Safety Navigator to support client navigation across the Integrated Care Center. The role includes welcoming patients, guiding them to services, and maintaining a safe environment for all visitors.

The PSN will de-escalate crisis situations, escort patients to appointments, and partner with staff to coordinate care, admissions, and transportation as needed.

Qualifications

• HS Diploma or GED required.
• CHW or mental health certificate strongly preferred.
• Familiarity with supportive counseling strategies and wellness principles in behavioral health.
• Knowledge of San Francisco community behavioral and public health systems.
• 2–4 years of lived experience with community behavioral health or criminal justice systems.
• Ability to work with diverse populations facing multiple barriers to education and employment.
• Strong communication, planning, and time management skills.

Responsibilities

• Welcomes and assists patients navigating the clinical resources throughout the building.
• Coordinates care and patient flow between HealthRIGHT360 programs.
• Escorts patients to appointments and provides service information.
• Maintains a safe, welcoming environment and de-escalates tense situations.
• Assists with admissions data collection and enrollment in services.
• Coordinates transportation for patients in residential programs.
