Job summary

AACI is seeking a Patient Navigator to support patients through the care continuum at our San Jose location. The role involves engaging patients, conducting needs assessments, and assisting with enrollment in insurance programs while coordinating care with the Health Center team.

The ideal candidate has 2+ years in patient navigation or case management in a healthcare setting, strong customer service, and proficiency with EHRs and Microsoft Office.

Qualifications

• Proven patient navigation or case management experience.
• Experience in a health care setting.
• Ability to communicate with diverse populations.
• Proficient with Microsoft Office applications.

Responsibilities

• Engage potential patients and schedule new patient appointments.
• Conduct assessments and survey patients to assess needs.
• Perform warm handoffs between providers and patients.
• Enroll patients into eligible insurance programs and assist with billing issues.
• Provide care coordination for AACI clients.
• Provide translation and interpretation during medical visits and referrals.
