Job summary

Reserve Care offers hospice, palliative and grief support in Ohio. The Care Navigation Nurse LPN supports referral intake, care coordination and initial clinical triage to ensure timely access to services.

The role emphasizes one-call resolution and smooth progression of referrals through dispatch. You will manage referrals from multiple sources, schedule appointments, follow up on stalled referrals, and assist with direct admits to inpatient centers.

Qualifications

• LPN license in Ohio.
• Hospice experience strongly preferred.
• Excellent verbal and written communication.
• Ability to communicate calmly and compassionately with patients and families.
• Strong teamwork and collaboration skills.
• Comfortable using EMR, phone systems and referral platforms.

Responsibilities

• Retrieve, monitor, and process referrals for hospice services and Navigator candidates from multiple sources.
• Learn and utilize the dispatch process to schedule appointments for prospective patients.
• Follow up on referrals that do not progress to dispatch in a timely manner.
• Perform direct admits to inpatient care centers as needed.
• Take clinical calls from patients and caregivers; attempt resolution and escalate as needed.
• Notify primary care teams of any clinical issues addressed.
• Participate in quality improvement and process improvement initiatives.
• Perform other duties as assigned.
