Job summary

Your Health Organization seeks a Clinical Scheduler to act as central scheduling and operational coordinator for the care team, ensuring the appropriate number and type of in-person encounters for patients in our value-based care model.

The role focuses on coordinating care with directors and providers, improving patient engagement and outcomes through precise scheduling and documentation, in a full-time Macon office setting.

Qualifications

• High school diploma or equivalent required; Associate's or Bachelor's in Healthcare preferred.
• CMA, LPN or RN preferred.
• Experience in medical scheduling/administrative role.
• Familiar with medical terminology and insurance verification.

Responsibilities

• Register, verify and schedule patients according to the care model.
• Track new patients until scheduled.
• Schedule appointments based on availability and urgency.
• Optimize schedules across disciplines.
• Coordinate discharge scheduling from inpatient facilities.
• Schedule TCM appointments within required windows.
• Maintain thorough documentation.
• Use multiple software systems to support follow-up.
• Communicate with patients via email, portal, phone, or in-person to schedule and instruct.
