We are seeking a Clinical Scheduler to fill our Care Manager role assist with all clinical scheduling needs for our Macon patients. The role serves as the central scheduling and operational coordinator for a single interdisciplinary care team, ensuring that patients receive the appropriate number and type of in-person encounters required under the Your Health proactive, value-based care model.
This position will be working in the Macon office daily. This is a full-time, salary-based8-hr position (8:00AM-5:00PM) (Monday-Friday).

Your Health is currently looking for a Care Manager to join our growing Primary Care family. A successful Care Manager is responsible for coordinating all in-person facilitator visits and in-person provider encounters for patients assigned to their care team. Through effective scheduling and coordination, the Care Manager ensures that patients receive consistent monitoring, engagement, and care team oversight. Your Health’s model of care is based on frequent patient engagement, coordinated interdisciplinary services, and proactive monitoring, which has consistently been shown to reduce hospitalizations and improve patient outcomes.
The Care Manager works closely with Directors of Population Health (DPH), providers, facilitators, and specialty population health divisions to ensure that clinical recommendations are translated into a clear and organized patient visit schedule.
