Your Health Organization is seeking a Clinical Scheduler to support our Care Manager team in Macon. This role coordinates all in-person visits for a single interdisciplinary care team to ensure appropriate patient encounters under our proactive, value-based care model.
Based in the Macon office, this full-time salary-based role from 8:00 AM to 5:00 PM Monday through Friday requires strong scheduling, organization, and communication skills to improve patient outcomes and reduce hospitalizations.

Your Health is currently looking for a Care Manager to join our growing Primary Care family. A successful Care Manager is responsible for coordinating all in‑person facilitator visits and in‑person provider encounters for patients assigned to their care team. Through effective scheduling and coordination, the Care Manager ensures that patients receive consistent monitoring, engagement, and care team oversight. Your Health’s model of care is based on frequent patient engagement, coordinated interdisciplinary services, and proactive monitoring, which has consistently been shown to reduce hospitalizations and improve patient outcomes.
The Care Manager works closely with Directors of Population Health (DPH), providers, facilitators, and specialty population health divisions to ensure that clinical recommendations are translated into a clear and organized patient visit schedule.
