Job summary

Your Health Organization in Macon is seeking a Clinical Scheduler to coordinate all in-person visits for the Care Manager team. This role ensures patients receive the appropriate number and type of encounters under the proactive care model in a daily Macon office setting.

It is a full-time salary position from 8:00 AM to 5:00 PM, Monday through Friday. As central scheduling and operations coordinator, you will work with the care team to optimize schedules, track patients, and support

Qualifications

• High school diploma or equivalent required; healthcare degree preferred
• Experience in medical scheduling or administrative role
• Familiar with medical terminology and insurance verification
• Strong organizational and time management skills
• Excellent communication and interpersonal abilities
• Ability to work under pressure

Responsibilities

• Register, verify, and schedule patients per care model
• Track and follow up with new patients until scheduled
• Schedule appointments based on care model and patient needs
• Optimize schedules across disciplines
• Follow patients after discharge from inpatient facilities
• Coordinate TCM appointments promptly within required windows
• Maintain strong documentation
• Use multiple software/tracking systems to manage follow ups
• Interact with patients via email, portal, phone, AthenaText, in person to gather info and provide pre-appointment instructions

What Are We Looking For

Your Health is currently looking for a Care Manager to join our growing Primary Care family A successful Care Manager is responsible for coordinating all in person facilitator visits and in person provider encounters for patients assigned to their care team Through effective scheduling and coordination the Care Manager ensures that patients receive consistent monitoring engagement and care team oversight Your Healths model of care is based on frequent patient engagement coordinated interdisciplinary services and proactive monitoring which has consistently been shown to reduce hospitalizations and improve patient outcomes The Care Manager works closely with Directors of Population Health DPH providers facilitators and specialty population health divisions to ensure that clinical recommendations are translated into a clear and organized patient visit schedule The following is a list of essential functions which may be subject to change at any time and without notice Management may assign new duties reassign existing duties andor eliminate functions

Areas of Responsibility

• Appropriately register verify and timely schedule patients according to the Companys care model
• Ensure new patients are being tracked and followed until scheduled
• Schedule appointments for patients according to care model including follow ups with healthcare providers based on availability medical urgency and patient preferences
• Ensure all disciplines with schedules are fully optimized
• Ensure patients are being followed and scheduled appropriately when discharged from an in patient facility
• Ensure TCM appointments are scheduled promptly and within the required window
• Ensure strong documentation is implemented
• A multitude of software systems including tracking systems will be utilized to ensure patient follow up and better patient outcomes
• Interact with patients via email patient portal telephone AthenaText in person etc to gather necessary information schedule and confirm appointments and provide pre appointment instructions

Qualifications

• High school diploma or equivalent required while an Associates or Bachelors Degree in Healthcare is preferredPreferred CMA LPN or RN
• Previous experience in a medical or healthcare setting preferably a scheduling or administrative role
• Familiar with medical terminology procedures and insurance verification purposes
• Strong organizational and time management skills with the ability to multitask and prioritize responsibilities effectively
• Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization
• Strong problem solving abilities
• Ability to work effectively under pressure
