Job summary

Your Health Organization is seeking a Care Manager to coordinate all in-person visits and provider encounters for patients assigned to the care team. You will manage scheduling to ensure consistent monitoring, engagement, and coordinated care across disciplines.

Reporting to population health directors, you will facilitate translations of clinical recommendations into organized visit schedules, using multiple software systems to optimize patient follow-up and outcomes.

Qualifications

  • High school diploma or equivalent; healthcare degree preferred.
  • Preferred CMA, LPN or RN.
  • Medical/healthcare experience, scheduling or administrative role.
  • Knowledge of medical terminology, procedures, and insurance verification.
  • Strong organizational and time management with multitasking ability.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively under pressure.

Responsibilities

  • Register, verify and schedule patients per the care model.
  • Track new patients and ensure timely scheduling.
  • Schedule appointments based on care model, urgency, and patient preferences.
  • Optimize all scheduling disciplines.
  • Follow patients after discharge and ensure timely rescheduling as needed.
  • Schedule TCM appointments within required windows.
  • Maintain strong documentation across systems.
  • Interact with patients via email, portal, phone and in person to gather information, confirm appointments and provide pre-appointment instructions.

We are seeking a Clinical Scheduler to fill our Care Manager role assist with all clinical scheduling needs for our Brunswick patients. The role serves as the central scheduling and operational coordinator for a single interdisciplinary care team, ensuring that patients receive the appropriate number and type of in-person encounters required under the Your Health proactive, value-based care model.

This position will be working in the Brunswick office daily. This is a full-time, salary-based8-hr position (8:00AM-5:00PM) (Monday-Friday).

What Are We Looking For?

Your Health is currently looking for a Care Manager to join our growing Primary Care family. A successful Care Manager is responsible for coordinating all in-person facilitator visits and in-person provider encounters for patients assigned to their care team. Through effective scheduling and coordination, the Care Manager ensures that patients receive consistent monitoring, engagement, and care team oversight. Your Health’s model of care is based on frequent patient engagement, coordinated interdisciplinary services, and proactive monitoring, which has consistently been shown to reduce hospitalizations and improve patient outcomes.

The Care Manager works closely with Directors of Population Health (DPH), providers, facilitators, and specialty population health divisions to ensure that clinical recommendations are translated into a clear and organized patient visit schedule.

The following is a list of essential functions, which may be subject to change at any time and without notice. Management may assign new duties, reassign existing duties, and/or eliminate function(s)

Areas of Responsibility:
  • Appropriately register, verify and timely schedule patients according to the Company’s care model.
  • Ensure new patients are being tracked and followed until scheduled.
  • Schedule appointments for patients according to care model including follow-ups with healthcare providers based on availability, medical urgency, and patient preferences.
  • Ensure all disciplines with schedules are fully optimized.
  • Ensure patients are being followed and scheduled appropriately when discharged from an in-patient facility.
  • Ensure TCM appointments are scheduled promptly and within the required window.
  • Ensure strong documentation is implemented.
  • A multitude of software systems (including tracking systems) will be utilized to ensure patient follow-up and better patient outcomes.
  • Interact with patients via email, patient portal, telephone, AthenaText, in-person, etc. to gather
    necessary information, schedule and confirm appointments, and provide pre-appointment
    instructions.
Qualifications
  • High school diploma or equivalent required, while an Associate's or Bachelor's Degree in Healthcare is preferred.
  • Preferred CMA, LPN or RN
  • Previous experience in a medical or healthcare setting, preferably a scheduling or administrative role.
  • Familiar with medical terminology, procedures, and insurance verification purposes.
  • Strong organizational and time management skills, with the ability to multitask and prioritize
    responsibilities effectively.
  • Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization.
  • Strong problem-solving abilities
  • Ability to work effectively under pressure.
