Job summary

Your Health in Savannah seeks a Clinical Scheduler to coordinate all in-person facilitator visits and provider encounters for patients on a single interdisciplinary care team, ensuring timely engagement and coordinated care under a proactive, value-based model.

This full-time role runs 8:00 AM–5:00 PM, Monday–Friday, in the Savannah office, and uses multiple software systems to support scheduling, documentation, and patient outcomes across facilities, homes, clinics, and virtual visits.

Qualifications

• High school diploma or better; healthcare degree preferred.
• Experience in medical/healthcare scheduling or administrative role is desirable.
• Familiarity with medical terminology, procedures, and insurance verification is helpful.
• Excellent organization and time-management abilities are essential.

Responsibilities

• Register, verify, and schedule patients according to the care model.
• Track new patients and ensure follow-up scheduling.
• Schedule appointments based on care model, availability, urgency, and patient preferences.
• Optimize schedules across all disciplines and coordinate discharges.
• Schedule timely TCM appointments within required windows.
• Maintain thorough documentation and use multiple software systems to support scheduling and follow-up.
• Communicate with patients via email, portal, phone, and in person to confirm appointments and provide pre-appointment instructions.
