Job summary

SAC Health in Indio, CA is seeking an ECM Care Manager, Behavioral Health to coordinate care for moderate to high-risk patients within an interdisciplinary team. You will provide brief behavioral interventions, link patients to community resources, and promote engagement in chronic medical and behavioral health care.

The role requires an MSW, a CA driver's license, ACSW preferred, EPIC certification, and 1+ year of case management experience.

Qualifications

• Master's Degree in Social Work (MSW) or related field is required.
• CA Driver's License and Auto Insurance are required; ACSW is highly preferred.
• Current CPR/BLS certification (AHA or Red Cross) and EPIC certification for the module you are hired into.

Responsibilities

• Provide intensive case management for a set caseload of patients with complex behavioral and medical needs.
• Conduct initial and ongoing behavioral health and psychosocial assessments; recommend plans based on assessments.
• Collaborate with interdisciplinary care team across the continuum of care.
• Participate in outreach to engage new patients and promote programs to patients, families, and staff.
• Meet productivity expectations set by management and prepare systematic case reviews with the team.
• Identify health/wellness goals with patients and incorporate them into shared care plans.
