Job summary

SAC Health is seeking an ECM Care Manager, Behavioral Health to clinically coordinate care for moderate to high‑risk patients in Indio, CA. The role includes brief behavioral interventions and linking patients to community resources, with collaboration across an interdisciplinary team.

Responsibilities include crisis assessment, documentation, patient education, and home visit duties as needed. Licensure and EPIC certification are used per scope of practice.

Qualifications

• MSW degree required with clinical background.
• ACSW certification preferred for advanced practice.
• Experience coordinating care for high‑risk patients is desirable.

Responsibilities

• Provide intensive care management for a caseload of patients with complex behavioral and medical needs.
• Conduct initial and ongoing behavioral health assessments and make recommendations.
• Coordinate care with interdisciplinary team and community resources to support goals.
• Engage new patients and promote programs to families and staff to build a patient base.
• Meet productivity targets and prepare/present case reviews with the team.
• Ensure care plans align with provider orders and patient wellness goals.
