Job summary

Independent Living Systems (ILS) is seeking a Care Manager in Saint Augustine, FL to coordinate medically necessary long-term care services with a focus on person-centered planning and seamless transitions.

You will assess needs, develop care and discharge plans, and collaborate with clinicians, families, and community resources to promote health, independence, and quality of life. Strong advocacy and documentation are essential.

Qualifications

• Minimum two years of relevant care management experience.
• Bachelor’s degree in social work, sociology, psychology, gerontology, or related field.
• RN license in Florida or LPN license with four years of practice in Florida.
• Proficiency in EHR and basic computer applications.

Responsibilities

• Serve as primary contact for members and authorized representatives.
• Assess needs and help develop person-centered care plans.
• Monitor progress and adapt plans to changing needs.
• Coordinate services across the continuum with providers and resources.
• Educate members on resources and self-advocacy.
• Maintain accurate documentation and ensure policy compliance.
• Collaborate with multidisciplinary teams to streamline care.
