Independent Living Systems (ILS) is seeking a Care Manager in Clearwater, FL to coordinate medically necessary long-term care services for eligible individuals. You will assess needs, develop person-centered plans, and work with a multidisciplinary team to ensure seamless transitions and optimal outcomes.

The role emphasizes advocacy, education, and collaboration with families and community resources to align care with guidelines and program requirements.

Formación

• Bachelor’s degree in a relevant social services field.
• RN or LPN licensure in Florida may substitute for degree requirements.
• Proficiency with EHR systems and basic computer applications.

Responsabilidades

• Serve as the primary point of contact for members and authorized representatives.
• Assess member needs and assist in developing a person-centered care plan.
• Monitor progress and adjust care plans to changing needs.
• Coordinate services across continuum and communicate with providers.
• Educate members on resources and self-advocacy.
