Job summary

Independent Living Systems (ILS) seeks a Care Manager to coordinate comprehensive long-term care for eligible individuals, ensuring coordinated, person-centered services that promote health and independence.

You will assess needs, develop care plans, coordinate services, and monitor progress, collaborating with healthcare providers, families, and community resources to optimize outcomes across the care journey.

Qualifications

• Bachelor’s degree in social work or related field required.
• RN or LPN licensure in Florida preferred depending on role.
• Experience with electronic health records (EHR) essential.

Responsibilities

• Serve as primary contact for members and authorized representatives.
• Assess needs, identify care gaps, and develop person-centered care plans.
• Monitor progress and adjust plans to ensure continuity of care.
• Coordinate services across the continuum and liaise with providers.
• Educate members on available resources and self-advocacy.
• Maintain documentation and ensure compliance with policies.
