Job summary

CNS Cares, LLC is seeking a Care Manager to oversee the delivery of coordinated patient care, maximize authorization units, and advocate for patient needs while ensuring satisfaction with highly responsive service.

The role requires scheduling staff, coordinating with DME suppliers, and liaising with nurses, physicians, and the claims team to ensure timely care and compliance with policies.

Qualifications

• High School Diploma and Associates Degree in a related field.
• At least two (2) years of experience in healthcare; or equivalent combination of education and EEOICPA experience.
• At least two (2) years of staff scheduling experience required.

Responsibilities

• Demonstrate regular attendance and punctuality; work with all levels of the Company; provide exceptional customer service.
• Schedule visits for the care team in the EMR and source staff to cover needs.
• Identify patient needs for durable medical equipment and coordinate with DME providers.
• Evaluate patient conditions and advocate for addition of conditions to DOL covered lists.
• Assist authorization team with starts of care and renewals to justify care levels.
• Attend patient appointments to assist physicians with the DOL Home Health Care program.
• Gather medical records as needed.
• Ensure staffing aligns with authorized hours and minimize overtime.
• Provide EMR and home orientation training to staff.
• Participate in an on-call rotation.
• Adhere to all company policies and perform other duties as assigned.
