Job summary

CNS Cares, LLC is seeking a Care Manager to schedule direct care staff for patient visits in a Lawrenceburg, IN setting. The role focuses on maximizing authorization units, advocating for patient needs, and coordinating with multiple entities for fast, accurate service.

You will communicate with nurses, the claims team, and leadership, ensuring patient satisfaction through impeccable service and compliant scheduling practices.

Qualifications

• Two years healthcare experience or equivalent combination of education and EEOICPA experience.
• Two years of staff scheduling experience required.
• Must be at least 21 years of age.

Responsibilities

• Schedule visits for the care team within the EMR and ensure 100% of authorized hours are utilized.
• Coordinate with recruiting and local leadership to source and hire staff.
• Identify patient needs for DME and coordinate with suppliers to meet those needs.
• Support authorization start of care and renewal periods to justify appropriate care levels.
• Attend patient appointments to assist physicians with the Home Health Care program.
• Gather medical records as needed and ensure scheduling complies with state guidelines.
• Provide EMR and home orientation training to staff.
• Participate in an on-call rotation.
• Adhere to all company policies and perform other duties as assigned.
