Job summary

The CNS Cares, LLC Care Manager will oversee coordinated care for patients assigned to our team, ensuring timely access, appropriate authorization units, and high patient satisfaction. You will advocate for patients, coordinate with nurses, home health aides, and claims teams, and work across departments to deliver impeccable service.

In this in-office role, you will schedule visits in the EMR, confirm staffing coverage, recruit local staff with leadership, and ensure all units are staffed

Qualifications

• High school diploma required with associates degree in a related field.
• Minimum 2 years healthcare experience or EEOICP/related experience.
• Minimum 2 years staff scheduling experience.

Responsibilities

• Coordinate care delivery for assigned patients and maximize authorization units.
• Schedule visits within the EMR and arrange staff coverage.
• Source and hire new staff with recruiting team.
• Ensure staffing meets authorized hours and compliance.
• Coordinate DME needs and assist patients with authorization.
• Attend medical appointments as needed.
• Gather medical records.
• Complete orientation training for staff.
• Participate in on-call rotation.
