Job summary

Mosaic Health, LLC. is hiring a Care Manager in Lakeland, FL to support patients and families through navigation, coordination of appointments, and access to resources.

You will collaborate with care teams, interpreters as needed, and community resources to ensure timely follow-up and smooth patient experiences. Responsibilities include coordinating referrals, tracking actions, communicating steps empathetically, and escalating complex barriers to licensed staff when needed.

Qualifications

• High school diploma or equivalent.
• 2-4 years of healthcare, patient navigation, care coordination or related experience.
• Experience with electronic health records, scheduling, or CRM systems.
• Strong problem-solving, documentation, and follow-through skills.
• Bilingual skills or community health worker training preferred based on patient population needs.

Responsibilities

• Coordinate patient access activities including appointment scheduling, referral follow-up, resource connection, and care team communication.
• Review patient information to identify barriers to care access.
• Resolve scheduling, documentation, or service coordination issues within procedures.
• Communicate next steps to patients and families clearly and empathetically.
• Track outreach, pending actions, and completed tasks in designated systems.
• Partner with clinical, operational, payer, and community resource teams for timely follow-up.
• Escalate clinical concerns or complex barriers to licensed clinical staff or leadership.
