Job summary

IMA Medical Group in Florida is seeking a Care Manager to support patients and families through routine and moderately complex navigation of care services. You will coordinate appointments, communicate with the care team, assist with community resources, and ensure timely follow-up across settings.

Qualifications

• High school diploma or equivalent required; 2-4 years of healthcare navigation, care coordination or related experience.
• Experience with appointment coordination, referrals, and patient communication standards.
• Working knowledge of scheduling, insurance basics, and care management workflows.

Responsibilities

• Coordinate patient access activities including appointment scheduling, referral follow-up, resource connection, and care team communication.
• Review patient information to identify common logistical, financial, social, or operational barriers to care access.
• Resolve moderately non-routine scheduling, documentation, or service coordination issues within established procedures.
• Communicate next steps to patients and families in a clear, empathetic, and culturally responsive manner.
• Track outreach, pending actions, completed tasks, and unresolved barriers in designated systems.
• Partner with clinical, operational, payer, and community resource teams to support timely patient follow-up.
• Escalate clinical concerns, complex access barriers, or situations outside established procedures to licensed clinical staff or leadership.
