The Ohio State University University Hospital Care Management Services seeks a Care Manager RN - Inpatient to guide an assigned group of patients from admission through discharge, coordinating pre-admission to post-discharge care and safe transitions.

The Clinical Care Manager collaborates with physicians, nurses and other health care workers to assess, plan, implement and evaluate care, monitor timeliness and resource use, and promote evidence-based practice.

Qualifications

• Bachelor's degree in Nursing (required).
• RN licensure in good standing.
• Two years of recent clinical experience preferred.

Responsibilities

• Coordinate inpatient care for an assigned group of patients from admission to discharge.
• Assist with pre-admission to post-discharge coordination of care.
• Collaborate with physicians, nurses and other health care workers to optimize care and outcomes.
