Qualifications

• Current State of Ohio RN license required.
• Bachelor’s degree in Nursing from an accredited program.
• Master’s degree preferred.
• CCM or similar certification desired.
• 2 years of relevant experience required.

Responsibilities

• Deliver superior customer service and achieve clinical and operational excellence.
• Manage a caseload of moderate to high‑risk adult and pediatric patients.
• Coordinate care across hospitals, SNFs, home health, hospice, and community services.
• Explain complex information to patients and families; build consensus on treatment plans.
• Assist with development and implementation of medical guidelines and policies.
• Document treatment plans and outcomes; perform utilization management.
