Job summary

Independent Living Systems (ILS) is seeking a Care Manager to coordinate comprehensive care for eligible individuals requiring long-term care services. The role involves assessing needs, developing person-centered care plans, coordinating services, and guiding transitions across care settings.

Qualifications

Bachelor’s degree in social work, sociology, psychology, gerontology, or related field.
RN or LPN license in Florida.
Minimum of two years of relevant care management experience.
Proficiency in EHR and basic computer applications.

Responsibilities

Serve as the primary point of contact for members and authorized representatives.
Assess member needs and develop a person-centered care plan.
Monitor progress and adjust care plans as necessary.
Coordinate services across the continuum and communicate with providers and community resources.
Provide education and support on available resources and self-advocacy.
Maintain accurate documentation and ensure compliance with policies and quality standards.
Perform other duties as assigned.
