Job summary

Qualifications

• At least 2 years of health care experience in care management or related setting.
• Valid and unrestricted driver's license and transportation.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office.

Responsibilities

• Complete assessments within regulated timelines to identify care coordination needs.
• Develop care plans with member, family, and providers to address needs and goals.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain ongoing member caseload for outreach and management.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration with care teams.
• Travel up to 25-40% as required by state/contractual requirements.
