Job summary

Molina Healthcare is hiring a Behavioral Health Care Manager in Florida to support care coordination across the continuum. You will work with a multidisciplinary team, develop care plans, conduct assessments, and monitor progress toward optimal member outcomes.

Ideal candidates have at least 2 years of health care experience, active licensure in behavioral health, a valid driver's license, and strong communication skills.

Qualifications

• Minimum 2 years health care experience, preferably in behavioral health.
• Active and unrestricted license in state of practice.
• Valid driver's license and reliable transportation.
• Experience with behavioral health, substance use, and foster care.
• Knowledge of whole person care principles and discharge planning coordination.

Responsibilities

• Provides support for care management and care coordination across the care continuum.
• Develops and implements care plans with member and network to address needs and goals.
• Performs telephonic, in-person or home visits as required.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of LTSS, behavioral health, and community resources for continuity of care.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and clinical guidelines to educate and motivate change.
• Assesses barriers to care and provides coordination support.
• May provide consultation and recommendations to peers.
• 25-40% travel may be required.
