Qualifications

• 2+ years in health care, care management or related field.
• Licensure/certification required only if state contract mandates.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and care coordination.

Responsibilities

• Complete member assessments per timelines and determine care coordination eligibility.
• Develop and implement care plans with member and care team.
• Conduct telephonic, home visits or in-person assessments as needed.
• Monitor care plan effectiveness and adjust as necessary.
• Maintain ongoing member caseload and perform outreach.
• Coordinate with LTSS, behavioral health and community resources.
• Facilitate ICT meetings and collaboration.
• Address barriers to care and assist members accordingly.
• Collaborate with licensed managers as required.
• Travel 25–40% may be required per state/contract.
