Job summary

Qualifications

• At least 2 years of health care experience including LTSS/care management.
• Experience with disability/chronic conditions is preferred.
• Driver's license and reliable transportation required for travel.
• Strong communication and written skills; ability to work with diverse populations.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines and during home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care team.
• Monitor care plans and document interventions and outcomes; adjust as needed.
• Promote integration of LTSS with behavioral health and community resources.
• Collaborate with ICT meetings for approval/denial of services and coordinate care.
• Travel 25–40% as required by state/contractual requirements.
