Job summary

Molina Healthcare is seeking a Care Management professional to coordinate member care across the continuum in Florida. You will assess needs, develop individualized care plans with the care team, and conduct visits as required.

The role involves ongoing monitoring, outreach to members, and collaboration with behavioral health, LTSS, and community resources. A valid driver’s license and strong communication skills are essential.

Qualifications

• At least 2 years of experience in health care, preferably care management, or equivalent education/experience.
• Valid and unrestricted driver’s license with reliable transportation.
• Ability to operate proactively and be detail-oriented.
• Excellent time-management and prioritization skills.

Responsibilities

• Completes assessments of members per regulated timelines and identifies eligibility for care coordination.
• Develops and implements care plans with the member, caregiver, physician and care team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plans, documents interventions and adjusts as needed.
• Maintains ongoing member caseload for outreach and management.
• Promotes integration of behavioral health, LTSS, and home/community resources.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing to educate and support change.
• Assesses barriers to care and assists members accordingly.
• Travel 25–40% may be required.
