Job summary

Monarch Healthcare in North Carolina is seeking an Extender to assist care managers delivering Tailored Care Management across physical health, I/DD, and TBI needs. You will engage with individuals, help set goals, and coordinate services to improve access to housing, healthcare, and community supports.

Requirements include a High School Diploma, Medicaid experience, and at least two years of related paid work in the Tailored Care ManagementEligible population.

Qualifications

• Direct experience providing care for and navigating the Medicaid delivery system on behalf of an individual.
• Lived experience with an I/DD or a TBI with knowledge of NC Medicaid delivery system.
• Two years of paid experience working directly with Tailored Care Management eligible population.

Responsibilities

• Perform general outreach, engagement, and follow-up with people we support to assist individuals in making informed care and service decisions.
• Provide a range of services to support safety, self-worth, confidence, connection to the community, and self-advocacy.
• Coordinate services and appointments, including reminders and transportation arrangements.
• Link individuals to community-based services for housing, employment, and healthcare access.
• Document encounters and submit billing documentation.
