Job summary

Piedmont Atlanta Hospital is seeking an experienced Care Management leader to coordinate team activities and drive high-quality social and transitional services for patients and families. The role focuses on discharge planning and transitions of care within the acute hospital setting.

Qualified candidates will have an RN license in Georgia (or multi-state licensure) and a Master’s in Social Work, with at least 2 years in care management or related fields.

Qualifications

• Associate’s Degree in Nursing required.
• Master’s degree in Social Work required.
• Georgia RN licensure and/or multi-state licensure required or LMSW licensure required.

Responsibilities

• Coordinates and monitors Care Management team activities, providing leadership, coaching, and mentoring to staff.
• Provides leadership and direction for Discharge Planning and Transitions of Care in the acute hospital.
• Monitors quality indicators to ensure appropriate social and transitional services for patients and families.
• Develops relationships with physicians, nursing supervisors, payers, and community resources to serve indigent, uninsured, and underinsured populations.

Work Experience

• 2 years in care management, medical social work or transitional care management Required

Licenses and Certifications

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure or
• LMSW - Licensed Medical Social Worker - State Licensure Required
