Job summary

Piedmont Healthcare is seeking a Care Manager to lead the Care Management team at Piedmont Atlanta Hospital, coordinating patient care across the continuum.

You will mentor and coach Care Management professionals, prioritize complex workloads, drive discharge planning, and partner with physicians, nursing leaders, payers, and community organizations to improve outcomes and transitions of care.

Qualifications

• Graduate of Nursing Program required.
• Master's degree preferred.
• 2 years of experience in an acute or post-acute setting required.
• Current unrestricted RN license in Georgia required.
• Certification in Case or Care Management preferred.

The Care Manager is responsible for care coordination, progression of care, and proactive discharge planning and is accountable for expediting the timely and safe discharge for all patients in their case load.

Qualifications:

Education

• Graduate of Nursing Program Required
• Master's degree Master's degree Preferred

Work Experience

• 2 years of experience in an acute or post-acute setting Required
• Experience using InterQual or Milliman Preferred

Licenses and Certifications

• Current unrestricted registered nurse (RN) license, or eligible for licensure, in the state of Georgia Required

Additional Licenses and Certifications

• Certification in Case or Care Management Preferred
