Job summary

Piedmont is seeking a Care Management leader to coordinate and mentor the Care Management team within a busy Atlanta hospital. The role focuses on discharge planning and transitions of care, ensuring quality social and transitional services for patients and families, and building strong collaborations with physicians, payers, and community resources.

The position requires an RN license in Georgia and a Master’s in Social Work, with at least two years of care management or related experience.

Qualifications

• Active Georgia RN license is required.
• Master's degree in Social Work is required.
• Two years of care management, medical social work, or transitional care management experience is required.

Responsibilities

• Coordinates and monitors all Care Management team activities, providing leadership, coaching, and mentoring to staff.
• Provides leadership and direction for Discharge Planning and Transitions of Care in the acute hospital setting.

Monitors for quality indicators to assure appropriate social and transitional services are provided to patients and families. Develops and maintains relationships with physicians, nursing supervisors, payers, community resources/ agencies to provide the needs services for indigent, uninsured, and underinsured populations.

Qualifications

Education

• Associate's Degree from accredited school of Nursing Required or
• Bachelor's Degree from accredited school of Nursing Preferred or
• Master's degree in Social Work Required

Work Experience

• 2 years in care management, medical social work or transitional care management Required

Licenses and Certifications

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure or
• LMSW - Licensed Medical Social Worker - State Licensure Required
