Job summary

Piedmont Cartersville is seeking an experienced Care Manager to coordinate patient care, monitor progression, and lead proactive discharge planning. You will be accountable for expediting timely, safe discharges for patients in your caseload and collaborating with the care team to ensure quality outcomes.

Required nursing licensure in Georgia and 2+ years in acute or post-acute settings. Experience with InterQual or Milliman is preferred, and a case or care management certification is a plus.

Qualifications

• Graduate of Nursing Program Required.
• Masters degree Preferred.
• 2 years of experience in an acute or post-acute setting Required.
• Experience using InterQual or Milliman Preferred.
• RN Licensure in Georgia Required.
• Certification in Case or Care Management Preferred.

Responsibilities

• Coordinate care across services to progress care.
• Proactive discharge planning to expedite timely and safe discharge.
• Manage a caseload to ensure timely progression of care and patient safety.
• Uphold nursing standards and collaborate with healthcare teams.
