Job summary

Piedmont Atlanta Hospital is seeking a Care Manager to lead care coordination, progression of care, and proactive discharge planning for a caseload of patients.

The role focuses on expediting timely, safe discharges while collaborating with physicians, nurses, and social work to ensure optimal patient outcomes in a fast-paced post-acute environment in Atlanta, GA. Must be a registered nurse with Georgia licensure, and experience with InterQual or Milliman criteria is preferred.

Qualifications

• Graduate of Nursing Program required.
• Master's degree preferred.
• 2 years' experience in an acute or post-acute setting required.
• Experience with InterQual or Milliman preferred.
• RN license in Georgia or multistate licensure required.
• Certification in Case or Care Management preferred.

Responsibilities

• Coordinate care and progression of care for assigned patients.
• Plan proactive discharge and expedite safe discharge.
• Collaborate with physicians, nurses, social work, and case managers to optimize patient outcomes.
