Job summary

Job description

The Manager of Care Management, under the guidance of the Director of Care Management, provides leadership, coaching and mentoring of the Care Management department professional and administrative staff to support and ensure standards of practice for service delivery and organizational performance strategies that align with PHC's Mission, Vision and Values. The manager will also provide Care Management services for hospital patients as appropriate to guide care facilitation, coordination, and transitions of care.

Education

• Graduate from a recognized, accredited school of nursing (Required)
• Bachelor's Degree in Social Work, Healthcare Administration, or other related field (Required)

Work Experience

• 5 years. If RN: Need combination of Nursing and Care Management experience in a hospital setting. Experience must include leadership/management experience in acute care setting (Required)

Licenses and Certifications

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure. If candidate is a nurse, then a current registered nurse (RN) license in the state of Georgia (or compact state) (Upon Hire Required)
• LMSW - Licensed Medical Social Worker - State Licensure. If candidate is a social worker, then a current unrestricted licensed master social worker (LMSW) licensure in the state of Georgia (Upon Hire Required)
• Certified Case Manager (Preferred)
• CSWCM - Social Work Case Manager Certification (Preferred)
