Job summary

Your Health is seeking a Clinical Scheduler to support our Care Manager in the Loris office. You will coordinate all in-person visits for the care team to ensure patients receive the right number and type of encounters under our proactive value-based model.

This full-time, salaried role runs 8:00 AM–5:00 PM, Monday to Friday, and involves scheduling across providers, facilitators, and the population health team to maintain timely patient care.

Qualifications

• High school diploma or equivalent required; an Associate's or Bachelor's in Healthcare preferred.
• Preferred CMA, LPN or RN.
• Experience in a medical or healthcare setting, preferably scheduling or administrative.
• Familiar with medical terminology, procedures and insurance verification.
• Strong organizational and time management skills with the ability to multitask.
• Excellent communication and interpersonal skills.
• Strong problem-solving abilities.
• Ability to work effectively under pressure.

Responsibilities

• Register, verify and timely schedule patients according to the Companys care model.
• Ensure new patients are being tracked and followed until scheduled.
• Schedule appointments for patients according to care model including follow ups with healthcare providers based on availability and urgency.
• Ensure all disciplines with schedules are fully optimized.
• Ensure patients are being followed and scheduled appropriately when discharged from an in-patient facility.
• Ensure timely scheduling of TCM appointments within the required window.
• Ensure strong documentation is implemented.
• Utilize multiple software systems to track patient follow up and improve outcomes.
• Interact with patients via email, patient portal, telephone, and in person to gather information, schedule and confirm appointments; provide pre-appointment instructions.
