Job summary

JEVS Care at Home in Pittsburgh seeks a Care Manager, Retention to develop and maintain client and caregiver relationships, ensuring retention and quality of care. The role manages long-term satisfaction, conducts home visits, EVV compliance, Plan of Care updates, caregiver evaluations, and incident reporting.

Qualifications

• Bachelor’s degree preferred or minimum 2 years’ experience in Home Care/Social Work.
• Proficient in Microsoft Office: Word, Excel, PowerPoint, Outlook.
• Knowledge of HHAeXchange, SAMS and/or HCSIS desirable.
• Experience with case management/coordination, records/utilization management, reporting desirable.
• Spanish speaking preferred and valid driver's license with own car.

Responsibilities

• Supervise a team of caregiver employees daily, including pre-billing, timesheet collection, and performance reviews.
• Monitor client eligibility, authorizations, and hours utilization for service delivery.
• Reconcile dashboard and pre-billing duties for weekly payroll processing.
• Care coordination – schedule, fill open shifts, ensure proper staffing.
• Fill open shifts to prevent disruption of client services.
• Ensure EVV and timesheet compliance.
• Monitor Plan of Care (POC) compliance.
• Complete home visits for all clients on caseload.
• Recruit new clients to meet budgeted goals and assist with marketing outreach.
• Provide on-call coverage as scheduled.
