Solvista Health is seeking a Care Manager to identify client and family needs, gaps in service, and help ensure those needs are met. You will work independently and in a team to de-escalate client issues, maintain professional demeanor, and accurately document scheduling and services.

The role requires a Bachelor’s degree in Human Services or related field and about 1 year of community services experience. Travel to community visits may be required, and safety protocols apply.

Qualifications

• Bachelor’s degree in Human Services or related field.
• 1 year of experience in Community services preferred.
• Ability to handle confidential information and work with diverse populations.
