Job summary

Volunteer-Behavioral-Health-Care-System is seeking a Care Manager to provide comprehensive care management for adults and children, focusing on strengths and integrating medical and behavioral health services. The role involves community outreach, care coordination with primary care, and ensuring access to social supports and resources.

Qualifications

• Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences and at least two (2) years of paid work experience in the behavioral health setting.
• Strong computer skills including use of Microsoft Word/Excel preferred.
• Experience in behavioral health settings is preferred.

Responsibilities

• Comprehensive Care Management – Initiate, complete, update, and monitor a comprehensive person-centered care plan as needed.
• Care Coordination – Participate in the patient’s physical health treatment plan and support appointment adherence.
• Referral to Social Supports – Identify and facilitate access to community supports and resources.
• Patient and Family Support – Provide high-touch in-person support and caregiver training.
• Transitional Care – Develop discharge plans and ensure timely follow-up care.
• Health Promotion – Educate patient/family on independent living skills and goals.
