Job summary

HomeWell Care Services, Inc. is seeking a Care Manager to coordinate, implement, and supervise the delivery of services for agency clients. The role ensures compliance with regulatory standards and maintains accurate client records in coordination with a small office team.

The ideal candidate has at least two years in home care settings, a valid driver’s license, strong communication and organizational skills, and a compassionate approach to supporting clients and families.

Qualifications

• Minimum of two years of experience in home care, home health, or a related field.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Excellent communication, organization, follow-through, and attention to detail.
• Strong problem-solving, judgment, and interpersonal skills.
• Ability to supervise caregivers, evaluate care needs, and advocate for clients and families.
• Compassion for helping others maintain dignity and quality of life.
• Professional demeanor and successful criminal background check.

Responsibilities

• Complete client assessments and required admission paperwork; provide copies of signed agreements.
• Develop and maintain individualized care plans with client or representative input and approval.
• Collect initial service fees in accordance with agency procedures.
• Introduce caregivers to clients, review care plans and expectations, and ensure care plans are accessible in the client’s home.
• Supervise and evaluate caregiver performance to ensure compliance with care plans and policies.
• Monitor changes in client condition, care needs, staffing skills, and availability of supplies.
• Maintain regular communication with clients and representatives to assess service quality and satisfaction.
• Conduct home visits, supervision visits, and reassessments as needed.
