Job summary

Molina Healthcare is seeking a Care Coordination professional to support care management and coordination across the continuum, collaborating with a multidisciplinary team to drive quality and cost-effective member care. You will complete member assessments within timelines, develop and implement care plans with the member and care team, and conduct telephonic, in-person, or home visits as needed.

Qualifications

• At least 2 years experience in health care, care management, or related setting.
• Licensed or certified as required by state contract.
• Valid driver license and reliable transportation for travel.
• Knowledge of community resources and coordination of services.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Assesses members per timelines and identifies care coordination needs.
• Develops and implements care plans with member, family, and care team.
• Performs telephonic, in-person, or home visits when required.
• Monitors care plan effectiveness and documents changes.
• Maintains ongoing member caseload and outreach.
• Promotes integrated services across behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings.
• Educates and motivates members using motivational interviewing.
• Identifies barriers to care and provides assistance.
• Collaborates with licensed care managers as needed.
• Travel up to 25-40% based on state requirements.
