Molina Healthcare, Inc. is seeking a behavioral health care coordinator to support care management activities across the continuum. You will work with a multidisciplinary team to develop and monitor care plans and ensure progress toward outcomes.

The role involves telephonic, face-to-face or home visits as required, 25-40% local travel, and collaboration to integrate behavioral health with LTSS and community resources. A valid clinical license and strong communication are essential.

Qualifications

• Minimum 2 years health care experience, preferably in behavioral health.
• Active license as listed (LCSW/LMSW/APSW/CHES/LPC/LPCC/LMFT or PhD/PsyD) and in good standing.
• Valid driver’s license, reliable transportation and adequate auto insurance for travel.
• Experience with severe mental health conditions, SUD, or foster care.
• Knowledge of whole-person care, chronic conditions, and discharge planning coordination.
• Data entry skills and experience using a clinical platform.
• Excellent verbal and written communication skills.
• Proficiency with MS Office or equivalent software.

Responsabilités

• Completes comprehensive behavioral health assessments within regulated timelines.
• Develops and implements care plans with the member and care team.
• Performs telephonic, in-person, or home visits as required.
• Monitors and documents progress and makes changes to care plans.
• Maintains ongoing member caseload and regular outreach.
• Promotes integration of services across behavioral health, LTSS and community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Uses motivational interviewing techniques to support change.
• Identifies barriers to care and provides appropriate assistance.
• May provide consultation to peers as needed.
• Travel 25-40% depending on state/contractual requirements.
