Job summary

Molina Healthcare, Inc. is seeking a licensed behavioral health clinician to provide support for care management and care coordination activities.

The role collaborates with a multidisciplinary team to deliver integrated member care and drive outcomes while contributing to cost-effective, quality services. The position involves comprehensive assessments, developing care plans with members and families, and facilitating visits by teleconference, in person, or at home.

Qualifications

• 2+ years healthcare experience, preferably in behavioral health.
• Active license in state of practice.
• Valid and unrestricted driver’s license with reliable transportation.
• Experience with severe mental health concerns, SUD, and foster care.
• Knowledge of whole person care, chronic conditions, and discharge planning.
• Data entry on clinical platforms.
• Excellent verbal and written communication.
• MS Office proficiency.

Responsibilities

• Complete behavioral health assessments per timelines and determine eligibility for care coordination.
• Develop and implement care plans with member, caregiver, physician and other professionals.
• Conduct telephonic, in-person or home visits as required.
• Monitor care plan effectiveness, document interventions, and adjust as needed.
• Maintain ongoing member caseload with regular outreach and management.
• Promote integration of behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing and Molina clinical guides to support change.
• Assess barriers to care and provide care coordination assistance.
• Provide consultation and resources to peers as needed.
• Travel 25-40% may be required.
