Job summary

Molina Healthcare is seeking a care manager to provide support for care management and care coordination activities across the continuum. You will collaborate with a multidisciplinary team to ensure progress toward outcomes and contribute to strategy for quality and cost-effective member care.

The role involves completing behavioral health assessments, developing and implementing care plans, conducting visits, monitoring plan effectiveness, and promoting integration of services.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician to include: LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD or PsyD; active and unrestricted license in state of practice.
• Valid driver's license, reliable transportation, and adequate auto insurance for job travel.
• Experience with persons with severe mental health concerns including substance use and foster care.
• Knowledge of whole person care principles, chronic conditions, and discharge planning coordination.
• Data entry skills and experience using a clinical platform.
• Excellent verbal and written communication skills.
• Microsoft Office suite proficiency.

Responsibilities

• Completes comprehensive behavioral health assessments per regulated timelines and determines eligibility for care coordination/case management.
• Develops and implements care plans in collaboration with member, caregivers, physicians and others.
• Conducts telephonic, in-person or home visits as required.
• Monitors care plans to evaluate effectiveness, documents interventions and suggests changes.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Assesses barriers to care and provides coordination to address concerns.
• May provide consultation to peers as needed.
• 25-40% estimated local travel may be required.
