Job summary

Community Care of North Carolina (CCNC) seeks a Care Manager to coordinate a full continuum of health care services for enrolled populations. The role collaborates with primary care providers, families, and the care team to develop and monitor SMART goals and personalized plans.

The position requires nursing or social work licensure, 2+ years of care management or related experience, and flexibility to travel across regions 2, 4, and 6 within North Carolina, including remote work within regions.

Qualifications

• Graduate from an accredited nursing program.
• Master’s degree in social work or related field preferred.
• Licensure requirements as determined by applicable board.
• Minimum 2 years’ social work or care management experience preferred.
• Access to Hospital/Data or EMR systems required.

Responsibilities

• Perform care management within RN/LSW scope of practice.
• Coordinate full continuum of health care services for enrolled populations.
• Conduct member assessments including medical, biopsychosocial, behavioral needs.
• Develop and evaluate member care plans with the care team.
• Provide education on diagnosis, medications, resources, prevention, and risk factors.
• Maintain documentation per policies and procedures.
• Travel within region/state as required.
• Utilize trauma-informed, motivational interviewing, strengths-based approaches.
